Meningitis Vaccination Information
Dear parents of Seniors,
If your child is planning to continue formal education after high school they will probably be required to have the following prior to attendance.
Meningitis (serogroup A,C,W,Y)
TB skin test $20.00 (insurance does not pay for this)
Meningitis B (highly recommended if attending a large university)
HPV (optional)
The Gove County Health Department will be at school Tuesday, April 9. If you would like for your child to receive any of these please return the following:

A signed permission form

A form of payment such as an insurance card ($20.00 for TB skin test)
If your child is uninsured, please contact the health department and they will help you with prices and financial assistance for these immunizations.
